Photo identification for you and your spouse (if filing a joint return)
Social Security cards or Individual Taxpayer Identification Number documents for you, your spouse, and all dependents
Birth dates for you, your spouse, and all dependents
If available, a copy of last year’s tax return (for reference)
All 1099 forms: 1099-G (unemployment), 1099-R (retirement income/pension payments), 1099-INT (interest), SSA-1099 (Social Security), 1099-NEC (self-employment)
If applicable, proof of health insurance (health insurance cards, MA 1099-HC
Any mail from the Internal Revenue Service (IRS). Most people do not have mail from the IRS.
Please remember to have the following tax and identification information/documentation with you on the day of your tax preparation appointment:
Any mail from the Massachusetts Department of Revenue (MA DOR). Most people do not have mail from the MA DOR.
If applicable, total child care expenses in 2024, and the child care provider’s name, address, and Employer Identification Number (EIN) or SSN
If applicable, all 1098T forms (tuition payments), receipts for materials and books, and bursar statement
If you want to receive your refund by direct deposit or want to pay your tax liability by ACH payment, your routing and account numbers.

Total rental expense paid in 2024
Total charitable contributions paid in 2024
All W-2 forms from all jobs for 2024
If you do not have all of the required documentation at the time of your appointment, we may may not be able to prepare and file your Federal and Massachusetts state tax returns.
